We would like to inform you that we are selling our Mall of America location to LensCrafters® effective February 01, 2022. All records for patients seen at Mall of America will be the property of LensCrafters® and we will no longer have access to the information without patient’s consent after January 31, 2022.

Your charts will continue to be at Mall of America. If you would like a copy of your records, please contact us at the Mall of America location.

Dr. Theresia Dixon, Dr. GaNe Boothroyd, and the rest of our doctors will be seeing patients at our other locations in Roseville, Ridgedale, and Burnsville. If you wish to continue your care with us, please fill out the record release form below and send to your preferred location.

Dr. Dixon will be scheduling patient’s with LensCrafters® at Mall of America semi occasionally for routine eye care.
